Please use the following procedures when completing medical forms for applicants:
1) SIGN across the applicant's photograph
2) SIGN and DATE the application form with your full medical title
3) Provide a business card / letter head with the correct phone,fax, address and title
4) Place a mark in each appropriate box for questions 1 - 7
5) Please provide a separate report if any abnormality is shown which
details diagnosis
management, and prognosis of the condition
6) Place all documents with an envelope and SIGN and DATE the sealed flap.
Please note that all applicants 16 year of age and over require a radiological report, however we only need the xray film sent to us if an abnormality is detected.
PLEASE NOTE THAT FORMS NEED MAY ONLY BE COMPLETED BY A MEDICAL DOCTOR (MD).
